Tailored Services

SPACS is a provider of support coordination for individuals living with a mental health and/or physical disability condition aged between 17 and 65 years.

SPACS is committed to ensuring people who have a physical and/or mental health disability integrate better in the community while feeling safe in the knowledge that they are being well-supported for all their needs.

Each Participant will have an individually tailored package of care specifically designed to support them to meet their goals, needs and preferences. SPACS will work in partnership with participants to understand what they want and hope to achieve, and then deliver services that meet those needs.

Bringing back focus & sharpening the mind

Our Support Coordination service works collaboratively to identify and coordinate appropriate supports for participants to meet their health goals and capacity building needs. Psychosocial recovery coaches support participants with psychosocial disabilities to live a full and contributing life. Recovery coaches will work collaboratively with participants, their families, carers and other services to design, plan and implement a recovery plan, and assist with the coordination of NDIS and other supports.

Soon, we intend to provide this service ourselves.

In the meantime, we assist you by ensuring you are linked with appropriate services where you can increase your skills and participate in community, social and recreational activities.  

This includes experiences that aim to improve your ability to interact with your surrounding community, from mental aspects such as raising self-esteem & body images to tangible aspects such as learning new skills & hobbies.


Coming back stronger

We support you to also engage with proper assessments and appropriate training to help you build new skills, find and keep a job, through disability employment services or other community further education.
